The key step in the creation of a transjugular intrahepatic portosystemic shunt (TIPS) is the needle puncture from a hepatic vein to a portal vein. This can be challenging in patients with portal vein thrombosis, where the portal vein is poorly visualized. Liwei Jiang, MD, discusses a feasibility study that employs a previously described technique of placing a guidewire into the right hepatic artery to serve as a landmark for the target portal vein.
